Do they smell like fish? I mean, ones of their benefits is supposed to be fresher breath. I'm hoping the fish flavor doesn't defeat that purpose.

They defeat bad breath by cleaning the teeth. They have that flavor (and subsequent smell) because they are still a treat that is supposed to be eaten and enjoyed and desired by cats. And yes they do help to clean cat's teeth and to reduce the bad breath. My baby one loves that and it is my choice for her.
